Mindfire (Disease). This creature can inflict the Mindfire disease: The creature's mind becomes feverish. The creature has disadvantage on Intelligence checks and Intelligence saving throws, and the creature behaves as if under the effects of the Confusion spell during combat.

Actions

Multiattack. The vrock makes two Diseased Talons attacks. It may replace one attack with a use of its Nightmarish Visions.

Diseased Talons. Melee Weapon Attack: +7 to hit, reach 5ft., one target. Hit 11 (2d6 + 4) slashing damage and 10 (3d6) poison damage.

Stunning Screech (1/day). The vrock lets loose an unholy screech. Each non-demon within 20 feet must make a DC 13 Constitution save. On a failure, the creature takes 28 (8d6) Thunder damage and is Stunned until the end of its next turn. On a success, a creature takes half damage instead.

Nightmarish Visions (Recharge 5-6). The vrock targets a creature that it can see within 30 feet and forces it to confront its deepest fears. The target must succeed on a DC 13 Wisdom save or become Frightened of the vrock for 1 minute (save ends at end of turn). While frightened in this way, the creature gains Bleeding [10 (3d6) psychic]. A bleeding creature suffers 10 (3d6) ongoing psychic damage at the end of each of its turns. A creature may use an action to attempt a DC 10 Medicine check to end the condition. The condition also ends if the creature receives any magical healing.

Toxic Breath (1/day). The vrock exhales toxic gas in a 15-foot cone. Each creature in that area must make a DC 15 Constitution saving throw. On a failure, the creature takes 38 (11d6) poison damage and is Poisoned for 1 minute (save ends at end of turn). If a creature fails this save three times, it becomes afflicted by Mindfire.
